• W2079551019 abstract "Determinar el significado clínico patológico de las calcificaciones intratesticulares.Se efectúa un análisis de calcificaciones intratesticulares detectadas en pacientes que consultaban por orquialgia infertilidad, o deformación intraescrotal en los últimos 10 años (febrero 1990-00), diagnosticadas por ecografía escrotal clásica de alta frecuencia, transductor de 6,5 Mhz. Se efectuó biopsia testicular en aquellos casos que presentaban modificaciones del tamaño de la gónada, además se realizó la determinación de marcadores tumorales clásicos. El hallazgo fue de 16 pacientes con microlitiasis sobre un total de 24 testículos con esta afección. Antecedentes previos: Infertilidad sin causa aparente: 6 pacientes; cirugía de devolvulación testicular y fijación contralateral en 5 casos; orquidopexia bilateral: 3 enfermos y epididimitis unilateral: 2 pacientes. Los pacientes fueron divididos en aquellos que presentan: calcificaciones periféricas o centrales. El periodo de seguimiento fue de 26,4 meses.Se detectaron como patologías concomitantes: a) Criptorquidia bilateral: 4 gónadas. b) Tumores de testículo: 5 gónadas. Estos tumores fueron biopsiados, resultando: 3 tumores seminomatosos, un carcinoma embrionario y un ca. in situ, ambos con marcadores negativos. La asociación con tumor testicular tuvo una incidencia de 20,83%. La localización central de las microlitiasis se detectó en los 5 tumores, mientras que periféricas fueron en 4 gónadas (criptorquídicas) y 15 difusas con aspecto de “tormenta de nieve”. Histopatológicamente: se observó cuerpos eosinofílicos, esféricos con núcleo calcificado con un predominio del cuadro histopatológico de la patología concomitante y no un cuadro propio.Es una patología de diagnóstico casual, cuyo significado clínico es aún un interrogante. Muestra una asociación con tumores testiculares del 20,83% y con otras patologías concomitantes en un 33%. Proponemos efectuar una biopsia testicular en aquellos casos que coexistan microlitiasis focales centrales y que tenga la gónada patología previa detectable.To evaluate the clinical-pathological significance of the intratesticular calcification.We analized by scrotal U.S., transducer 6.5 Mhz, the patients that consulted about testicular painful, infertility or intraescrotal deformity. Biopsy was effectued when detected size modification of testicles, and central or focal calcifications. Also was done the classics tumoral markers. We founds 16 patients with microlithiasis over 24 testicles with these pathology. Previous antecedents: infertility 6 cases, testicular devolvulations surgery 5c, bilateral orchidopexy 3c and unilateral epididymitis 2c. The calcifications was classified in: peripheric or central follow-up 26.4 months.Associated pathology: a) Bilateral cryptorchidism: 4 testicles. b) Testicular tumors: 5 testicles. These tumors were biopsed: 3 seminomas, 1 embrionary carcinoma and 1 ca in situ. Both with negative biologic markers. The association with testicular tumor had an incidence of 20.83%. The central localization was detected in the 5 tumors, while the peripheric were 4 testis (cryptorchidism) and 15 with aspect of “snow storm”. Histopathologically was observed eosinophilic bodys, with calcified nucleos, but not a exclusive features.Is a benigne condition, casual, whose clinical significance is a still enigma. The association with testicular tumor is 20.83% and associated pathology is 33%. We proposed a testicular biopsy in cases of microlithiasis focal, central or with previous pathology." @default.
