- W2079658387 abstract "The advent of effective blastocyst culture and vitrification has allowed significantly increased implantation rates in frozen embryo transfer (FET) cycles. However, not all embryos reach the appropriate stage for vitrification on the same day of culture (Day 5, Day 6 or Day 7). Extending culture to a seventh day is not performed routinely and therefore FET pregnancy outcomes from these embryos are still being investigated. Our retrospective data analysis from a single large private fertility center was devised to evaluate the FET implantation potential of the warmed blastocysts based specifically on the culture day they were originally vitrified. FET cycles from 2010-2013 (n=1273) were analyzed for the culture day the blastocysts were originally cryopreserved. All cases that had genetic analysis were excluded. Only cycles in which all of the embryos transferred were from the same original day of vitrification were included (Day 5=577, Day 6=548, Day 7=21) allowing for a “pure” analysis of implantation rates for each embryo culture day. Analysis for significance was performed by χ2 with Yates correction. Implantation rates significantly declined in FET cycles as the original culture day of blastocyst vitrification increased: 47.3% for Day 5, 28.4% for Day 6 and 11.1% for Day 7 (p<0.0001). The mean ages of the FET groups were not significantly different: 36.8 for Day 5, 37.0 for Day 6 and 37.2 for Day 7. Blastocysts that do not reach vitrification stages/grades on culture Day 5 have significantly reduced implantation potential in FET cycles. These findings demonstrate that when implantation can be analyzed in ”pure” FET cycles that embryo developmental delay results in lower success, even when an embryo reaches a similar stage/grade for subsequent vitrification on Day 6 or 7. Physicians and patients can now be counselled that although they may have embryos vitrified on Day 6 and 7, that these delayed blastocysts do not have the same pregnancy potential." @default.
