In the course of caring for patients with life-threatening or life-limiting illness, the challenge of how to effectively and safely deliver vital medications for symptom control often arises. Clinicians are often faced with a clinical conundrum when the oral route of delivery is no longer available for medication administration. The availability of compounded medications has begun to address these difficult clinical situations. A critical review of the scientific evidence to support the use compounded medications is needed in daily practice. The current state of the science will be reviewed about utilizing commercially available pharmaceuticals through alternative dosage formulations. A review of the options for alternative routes of delivery will proceed from the most desirable/least invasive and expensive to the least desirable/more invasive and expensive. This will include sublingual/buccal, intranasal/inhalational, topical, rectal, subcutaneous/intraosseous, and neuraxial delivery. A brief discussion of future directions for research in this area will be combined with polling the workshop participants about their most pressing daily needs in this area of practice. This workshop is designed to provide the participant with practical knowledge to deal with real, everyday clinical challenges in utilizing alternative routes of delivery for medications to maintain essential symptom control in the palliative medicine and hospice settings, and will consist of short didactic presentations on background information combined with real case-based learning. Participants who register for the course will be asked to provide one example of a clinical situation where an alternative dosage formulation and/or route of delivery was needed to the faculty prior to the workshop. The faculty will select a number of these cases as the basis for discussions and the development of a plan of care." @default.
