- W2079794312 abstract "Dementia with Lewy bodies (DLB) is claimed to be the second most common neurodegenerative dementia after Alzheimer's disease (AD). The core clinical features are visual hallucinations, fluctuating cognition and parkinsonism. Besides these criteria there are a number of features that are considered suggestive or supportive of a DLB diagnosis, for example autonomic dysfunction. One aspect of autonomic dysfunction is orthostatic hypotension that is known to be a common problem amongst patients with DLB. In this study we analysed the orthostatic blood pressure reaction in 235 patients with AD, 52 patients with DLB and 62 controls. The blood pressure and pulse rate were recorded after ten minutes rest in supine position, immediately after standing up and after 1, 3, 5 and 10 minutes of standing. A patient was considered to be orthostatic if the systolic blood pressure reduction was at least 20 mm Hg or the diastolic blood pressure reduction was at least 10 mm Hg. We found that 13% of the controls, 42% of the AD patients and 69% of the DLB patients (p<0.001 controls vs. AD and controls vs. DLB, p=0.001 AD vs. DLB) had an orthostatic blood pressure reaction. Moreover, the DLB patients had a greater drop in blood pressure upon standing and maintained a lower blood pressure throughout the ten minutes of standing than the patients with AD. The mean blood pressure values with standard deviations for the orthostatic DLB patients (n= 36) were 150±22/81±10 after ten minutes of rest in supine position and 112±25/68±15 on standing. After ten minutes of standing the blood pressure was 123±26/73±19. In comparison the orthostatic AD group (n=99) had a resting blood pressure of 154±25/86±13, 133±30/79±16 on standing and 139±25/84±14 after 10 minutes of standing. The difference were significant on standing and after 10 minutes of standing (p<0.01). The pulse reaction did not differ between the groups. In conclusion the study showed that the patients with DLB had a more severe and long standing blood pressure reaction to orthostatic challenge. This is important both for diagnosis, treatment and for understanding the patophysiology of the disease." @default.
