The actual presentation of these men to the emergency room (ER) has not been studied in detail. METHODS The Nationwide Emergency Department Sample (NEDS) is the largest all-payer ER database in the United States and contains between 25-30 million records from ER visits for >950 hospitals, thereby approximating a 20% stratified sample of US hospital based ERs. We evaluated a weighted estimate of 15,367 patients with mCaP who presented to the ER from January 1, 2006 - December 31, 2009 and studied the five most common ER diagnoses in these patients. RESULTS The most common diagnoses for ER visits in mCaP patients were hematuria (17.6%), urinary tract infection (UTI, 16.8%), acute renal failure (ARF, n=16.1%), hydronephrosis (15.8%), and pneumonia (6.5%) (Table 1). While the national frequency of hematuria remained stable from 2006-2009, the frequencies for each of the other diagnoses are increasing. A substantial majority of patients presenting with these diagnoses were admitted to the inpatient setting (Range: 93.7% for hematuria to 99.1% with ARF). Importantly a significant proportion of these patients died as an inpatient (Range: 4.5% for hydronephrosis to 18% for Pneumonia). CONCLUSIONS The most common diagnoses for ER visits in mCaP patients were hematuria, UTI, ARF, hydronephrosis, and pneumonia.
