- W2080914097 abstract "The concept of cognitive reserve (CR) is proposed to account for the clinical observation that highly educated individuals appear to cope better with the onset of dementia. The purpose of this study was to investigate how a measure of educational attainment modifies the relationship among cognitive decline and cerebral metabolism in amnestic mild cognitive impairment (MCI). The subjects were 77 patients with amnestic MCI showing cerebral metabolic pattern of normal or Alzheimer's disease (AD) in SEAD-J (Study on diagnosis of early Alzheimer's disease -Japan). They were divided into subcategories by AD conversion in three years (converter (MCI-con) or stable (MCI-sta)) and educational attainment (HiE (years of school education>=15) or LoE (years of school education <15). FDG PET images were analyzed with SPM. Cognitive decline was compared between subcategories of HiE and LoE using scores of MMSE, ADAS, WMS-R logical memory I (LM-I), and logical memory II (LM-II). MCI-HiE showed lower cerebral metabolism (rCMRglu) than MCI-LoE in the left parieto-temporal association area, left frontal association area, bilateral medial frontal areas, and bilateral posterior cingulate cortices, although there is no significant difference in age, GDS, MMSE, ADAS, LM-I, and LM-II . The voxel-based regression analysis between rCMRglu and the years of school education, controlling for the age and ADAS score, revealed a significant inverse association in the left parietotemporal association area, left medial temporal lobe, bilateral medial frontal areas, and bilateral posterior cingulate cortices (P<0.05). No significant difference in conversion rate was observed between MCI-HiE and MCI-LoE. MCI-con-HiE and MCI-con-LoE represented almost the same trend of cognitive decline in MMSE, ADAS, and LM-I. Significant lowered rCMRglu was seen in the frontal lobe in MCI-con-HiE than in MCI-con-LoE. MCI-sta-HiE demonstrated significantly faster decline in ADAS than MCI-sta-LoE (P<0.05), and significant decreased rCMRglu in the bilateral posterior cingulate than MCI-sta-LoE (P<0.05). The results suggest that CR provides a compensating mechanism, which allows high educated people to cope better with neural damage in MCI due to AD than low educated people. The rate of cognitive decline may differ based on the subjects' CR and status in disease progression of AD." @default.
