- W2081608285 abstract "OBJECTIVE: The impact of GnRH agonist long compared with GnRH antagonist protocols under in in vitro fertilization (IVF) conditions on endometrial receptivity is still debated. The aim of this study is to compare the effect of GnRH antagonist and agonist long protocols on the endometrial receptivity under controlled ovarian stimulation hyperstimulation (COS) compared with the natural cycle in the same patients. DESIGN: For the same normal-responder patients (n=30), endometrial biopsies were collected during the pre-receptive (LH+2) and receptive stages (LH+7) of a natural cycle and, subsequently, on oocyte retrieval day (hCG+2) and on transfer day (hCG+5) of a stimulated cycle either with GnRH agonist long or GnRH antagonist protocols. The messenger RNA (mRNA) content of each biopsy was analyzed by DNA chip microarray to evaluate the mRNA expression profile during the endometrial receptivity transition. MATERIALS AND METHODS: The mRNAs extracted from endometrial biopsies were analyzed on Affymetrix HG-U133 plus 2.0 GeneChip oligonucleotide microarrays. RESULTS: Both protocols affect endometrial receptivity in comparison with their natural cycle. Major differences in endometrial chemokines and growth factors in stimulated cycles in comparison with natural cycles were observed, associated with gene expression alterations of endometrial receptivity genes. However, the endometrial receptivity under GnRH antagonist was closer to the natural cycle receptivity than under GnRH agonist protocols. In addition, we observed the expression of endometrial cancer and endometriosis biomarkers during the endometrial receptivity under GnRH agonist long protocols, such as CXCL5, CXCL8, EGF, IGF1R, IGF2BP3 and MET. CONCLUSIONS: GnRH antagonist mimics more closely the natural endometrial receptivity than GnRH agonist long protocols. The GnRH agonist long protocol as a first choice should be reconsidered." @default.
