- W2081768223 abstract "The shortage of donor organs is a major limiting factor for transplantation. Less than 15-20% of multiorgan donors become lung donors. The lung transplantation (LTx) program gathers strength in Russia. So the demand for donor lungs has increased in recent years. The purpose of our study was to investigate the efficiency of donor lung utilization and limitations in the donor selection process. 142 brain-dead donors were registered in the Moscow Coordinating Centre of Organ Donation (MCCOD) for 2-years period. We examined lungs from 68 brain-dead patients from May 2011 to May 2013. There were 51 males and 17 females with mean age 37,0±8,4 years. Collected variables consisted of age, sex, ABO blood group, smoking history, partial pressure of oxygen fraction of inspired oxygen ratio, cause of death, duration of mechanical ventilation (MV) and stay in the intensive care unit (ICU), data of chest x-ray and bronchoscopy. We used the 4-step algorithm for the donor evaluation. Only 15 were considered suitable for lung donation. The most common cause of brain death was cerebral injury (70,6%). Main methods for diagnosing pathologic condition of donor lungs were chest x-ray and bronchoscopy (52,9%). The infection and aspiration of gastric contents or blood were the principal causes for refusal (54,4%). Risk factors for pulmonary infection were length of MV and donor hospitalization in ICU, aspiration, age and smoking history. The main risk factor for aspiration was the time of endotracheal intubation. Only 11 (16,2%) donor lungs were used for LTx. The utilization rate of donor lungs is 7,7%. The characteristics of utilized donor lungs were: mean partial pressure of oxygen 484,8 mm Hg, mean length of MV 37,3 hours, mean age 29,7 years. An efficient utilization system is needed to improve LTx in Russia. The management of multiorgan donor in the ICU and usage of the 4-step algorithm for donor evaluation may improve the utilization of donor lungs." @default.
