- W2082016228 abstract "To assess the diagnostic accuracy and tolerability of sonohysterography (SHG) with endometrial sampling (SHGes) compared to operative hysteroscopy with directed endometrial biopsy (HYSeb) in patients with abnormal uterine bleeding (AUB) and diffuse lesions at SHG. Seventy-two consecutive patients with AUB, endometrial thickness > 4 mm at TVS (Technos MP, Esaote, Genova, Italy), and diffuse lesions at SHG were prospectively recruited. SHG was performed with a 4.7 mm intrauterine catheter. In all patients an endometrial biopsy was performed by a syringe vacuum aspiration at the end of SHG. Hysteroscopy was performed, without any anesthesia, by using the Bettocchi continuous-flow operative hysteroscope (3.9 × 5.9 mm) (Storz), with forceps used to achieve directed endometrial biopsies (HYSeb). Endometrial specimen area (mm2) obtained by SHG, and hysteroscopic guided direct biopsy were compared. Pathologic reports were compared to the standard results of extended curettage. Mean age (±sd) was 41 years ± 12.79% of patients were in pre-menopause, 21% were post-menopausal. Forty-one patients (57%) were pluriparous. SHGes and HYSeb were successfully performed in all patients. SHGes was inadequate for histologic examination in two cases. All malignant lesions were correctly assessed by both techniques. Mean area (±sd) of endometrial specimen at SHGes significantly larger than one single sample at HYSeb, and not significantly different from three HYSeb samples (34.0 mm2 ± 26.9 vs 22.6 mm2 ± 33.7) (P = n.s.). Patients reported moderate and severe pain at SHGes and at HYSeb in 22%, and in 24% of cases, respectively (P = n.s.). Sonohysterography with endometrial sampling proved to be as accurate and tolerable as HYSeb. Both these imaging and biopsy techniques should be considered a reliable outpatient office procedure in the management of patients with AUB." @default.
