- W2082039912 abstract "T HE RISK of infection is increased during maintenance hemodialysis (MHO) treatment. 1 One well recognized risk factor for infection is the vascular access. 2 It provides a direct portal of entry to the circulation, and a local site favorable for bacterial colonization. Another potential risk factor for infection is altered immune function. 3 Immune alterations have been recognized for sometime in patients with chronic renal failure, and in the past these alterations have usually been attributed to uremia. 4•5 With the recent application of more sophisticated tests of immune function, an increasing variety of immune alterations have been documented in MHO patients, and predisposing causes other than uremia are being sought to explain these alterations. Protein-calorie malnutrition may be one such cause; it is associated both with altered immune function and an increased risk of infection. 6.7 Recently malnutrition has been recognized as a relatively common complication in MHO patients.8 Its recognition has been facilitated by a wider appreciation of the importance of nutrition in a variety of chronic diseases, by a more agressive approach to treatment, and by earlier diagnosis as a consequence of the more routine use of standardized nutritional assessment criteria. 9 In addition, malnutrition is probably more prevalent in MHO patients today than in their counterparts of several years ago; one reason for this is that the acceptance criteria for MHO treatment in patients with end-stage renal disease (ESRO) have gradually broadened since federal financial support became available in 1973.
