- W2082370680 abstract "Objective: In patients with pheochromocytoma high catecholamines exposure may lead to pronounced target organ damage. The aim of the ongoing study was to evaluate early target organ damage in patients with pheochromocytoma in comparison with patients with never treated essential hypertension and patients with OSA being characterized by increased sympathetic nervous system activity. Design and Method: We examined prospectively 20 patients with pheochromocytoma (mean age 45 ± 14 years, 11 M, 6 F) and 17 age and gender matched patients with essential hypertension(EHT) - mean age 38 ± 6 years, 11 M, 6 F. We also investigated group of 13 patients with obstructive sleep apnea (OSA) - mean age 41 ± 8 yrs, 11 M, 2 F. Echocardiography (LVMI), Doppler duplex examination of renal arteries (RI and PI were calculated) and carotid ultrasound followed by a duplex and colour Doppler examination were performed. Endothelial function was assessed by means of plethysmography (Endo-PAT 2000). Results: Pheochromocytoma patients were characterized by non-significantly worse endothelial function (1,9 ± 0,6 vs. 2,1 ± 0,6 and 2,2 ± 0,5) as compared with EHT and OSA patients. In 13 patients with pheochromocytoma in whom endothelial function was assessed after surgery no significant change in endothelial function was observed. Patients with pheochromocytoma as compared with EHT pts were characterized by more pronounced IMT (0,80 ± 0,26 vs. 0,66 ± 0,12 mm; p = 0,06), higher RI (0,63 ± 0,06 vs. 0,60 ± 0,04; p = 0,1) and higher LVMI. There was no difference in IMT, RI and LVMI between pheochromocytoma and OSA patients. There was a significant decrease in LVMI and nonsignificant in IMT in pheochromocytoma patients after surgical removal of the tumor as compared with baseline. Conclusions: Our study showed that patients with pheochromocytoma had more pronounced early target organ damage as compared with age and gender matched patients with essential hypertension - in contrast no difference was found between pheochromocytoma patients and subjects with OSA. Surgical removal of pheo was associated predominantly with regression of LVMI but not with marked improvement of structural and functional alterations of the vessels." @default.
