- W2083005748 abstract "To evaluate the impact of target volume re-delineation and re-planning based on interim CT simulation and fused interim 18FDG PET scan (taking advantage of tumor regression during chemo-radiotherapy) on local tumor control and normal tissue complication in non-small-cell lung cancer. Fourteen consecutive patients with locally advanced or inoperable non-small-cell lung cancer (10 stage III) who met the following criteria were selected for the process. A–Target volume delineation was based on fused PET-CT imaging according to method of Hong et al. (1) B–Even with the best 3DCRT plans PTV dose could not exceed 50 Gy due to excessive lung toxicity. C–Concurrent chemoradiotherapy. All patients were re-imaged/simulated (PET-CT) at 45–50 Gy (with concurrent chemotherapy). New target volumes were delineated and second set of plans were developed and used for treatment. A composite plan (combination of the first and second set of plans) was reviewed prior to treatment. All patients were treated to the average initial prescription dose of 47.6 Gy (range 45–50.4 Gy) and composite prescription dose of 65.9 Gy (range 57.6–73.8) without any increase in lung toxicity. The Initial and composite plan values for mean lung dose and V20 were 11.18 Gy, 22% and 14.42 Gy, 29%, respectively. Local control was achieved without any confirmed radiographic failure in 12 patients, one did not complete radiotherapy and was lost to follow up, and one progressed locally. Process of interim re-scanning with PET & CT and re-planning resulted in converting a clinically unacceptable therapeutic ratio (TR) to a very favorable outcome.
