This is especially true in the elderly and in medically complex patients, patients who prefer to avoid potentially morbid surgical treatment. Small renal masses (SRMs) can be approached with nephron-sparing surgery, radical nephrectomy, tumor ablation, or active surveillance depending on tumor features, patient characteristics, and patient preferences. To clarify the most appropriate treatment options and recommendations, patients and providers must weigh the patient’s projected longevity, the threat posed by the cancer to shorten this longevity, the effectiveness of available treatments, and the associated morbidity of the treatment choices. For an accurate discussion with our patients, it is vital that we correctly understand the threat of SRMs. When evaluating renal mass patients in clinic, the decision to perform surgery, both partial and radical nephrectomy, or to undergo a period of active surveillance is heavily dependent on tumor size. It has been observed that presenting tumor size represents a significant predictor of synchronous and asynchronousmetastases [1–3]. During surveillance, SRMs have demonstrated rates of growth between 2 and 3 mm/yr, and 30% of the SRMs demonstrated no growth over a 3-yr observational period [4]. The ability to determinemalignancy based on growth rates over time is debatable [4,5]. Thus presenting tumor size and subsequent growth rate are of the utmost importance in the discussion of treatment options, especially concerning management that optimizes nephron sparing at presentation or future intervention.
