- W2083211681 abstract "Abstract The effects of various trauma related factors on admission serum insulin (I) and glucose (G) levels were evaluated in 139 injured persons admitted directly from the scene of accident without previous treatment and in 108 persons transferred from other hospitals. Compared with the mean value of 27.9 μU/ml in 47 healthy controls, patients from the scene had insulin levels lower by 1.6% and glucose higher by 106.7% with I G ratio lower by 41.5% (34.6 ± 7.4 vs. 20.2 ± 12.4 μU/mg, P I G of 26.0 and 17.8 μU/mg, respectively. In persons from the scene the I value rose spontaneously from 26.0 ± 9.0 at 60 min to 34.2 ± 18.7 μU/ml at 150 min with little change in G levels. In 40 patients with hypotensive shock the mean I value was 36.7 μ/ml and was markedly dependent on the shock severity. Patients who died in shock had I G 10.0 ± 8.2 μU/mg compared with 21.3 ± 18.1 μU/mg in those who recovered from shock and survived. We conclude that severity of injury and presence of shock are more important factors in I secretion than the type of injury. The I G ratio is a more useful index than I or G alone. It may indicate whether the patient is in the state of depression of insulin secretion or in the state of peripheral insulin resistance. The I G ratio therefore, should be used as one of metabolic indices for severely injured individuals, rather than glucose or insulin alone. New automated radioimmunoassay systems make such a goal feasible." @default.
