- W2085202234 abstract "African Americans have a higher prevalence of cardiovascular disease than Whites. Several previous studies have demonstrated impaired flow mediated dilation of the brachial artery (FMD), a measure of endothelial function, in African Americans. Whether this ethnic difference in FMD is explained in part by decreased reactive hyperemia or attenuated vascular smooth muscle cell responsiveness to vasodilator stimuli is not well-established. To further evaluate ethnic differences in FMD, we measured FMD, hyperemic flow, and nitroglycerin-mediated dilation in 51 African Americans and 84 White middle-aged, healthy individuals. African Americans tended to be younger (38 ± 10 versus 42 ± 11 years, p = 0.063) and have a greater body mass index (28.6 ± 5.5 vs 26.7 ± 5.2 kg/m2, p = 0.043) than Whites. The groups were similar in gender composition (47% vs 45% female), 24 hour ambulatory systolic blood pressure (126 ± 12 vs 127 ± 15 mmHg), and LDL cholesterol (125 ± 35 vs 122 ± 34 mg/dl). Baseline brachial arterial diameters were similar in African Americans and Whites (4.4 ± 0.8 vs 4.2 ± 0.8 mm). FMD, however, was significantly lower in African Americans (2.7 ± 3.6 vs 4.3 ± 5.0 %, p = 0.042). This difference remained statistically significant after adjustment for baseline arterial diameter, age, and body mass index (p = 0.037). There were no ethnic differences in the ratio of hyperemic to resting blood flow (6.80 ± 3.19 vs 7.07 ± 3.81), in the mean shear stress induced by reactive hyperemia (61.5 ± 26.7 vs 65.3 ± 26.8 dyne/cm2), or in nitroglycerin-mediated dilation (13.3 ± 9.0 vs 14.0 ± 7.6 %). These results suggest that African Americans have smaller values for flow-mediated dilation of the brachial artery than Whites, and that this difference is not explained by decreased reactive hyperemia or attenuated vascular smooth muscle cell responsiveness to vasodilator stimuli." @default.
