• W2085698100 abstract "Objective: The aim of this study was to assess the onset of prostatic volume increase in Japanese men with BPH, which was proven histologically. Methods: Between 1990 and 2002, 68 Japanese men underwent repeated prostatic biopsies (sextant and one core from far lateral peripheral zone bilaterally) in our institution due to elevated serum prostate specific antigen and/or abnormal digital rectal examination. All the repeated biopsies revealed BPH histologically in all cases. Total gland volume (TGV) as well as transition zone volume (TZV) were measured by a 3-dimensional method using transrectal ultrasonography. The measurements for serum prostate-specific antigen (PSA) as well as these volumes were performed at the interval of 6 to 24 months. We stratified the 3 groups (G1: increase, G2: no change, and G3: decrease) based on the change of TGV and TZV. Results: Overall, 44% of the men had lower urinary symptoms, as measured by the IPSS symptom score. The age of the first biopsy ranged from 52 to 87 years old (average: 71). The numbers of repeated prostate biopsy, followed-up periods, and volume determinations in these 68 men ranged from 2 to 5 (average 2.2), from 1.5 to 8 years (average 3.5), and from 2 to 12 (average 4.8), respectively. The numbers for G1, G2, and G3 were 21, 44, and 3, respectively. Ages of the first volume determination on G3 were greater than 80. There were significant differences between G1 vs G2 and G3 in the age of the first volume determination (G1: 67.0 for TGV and 67.2 for TZV, G2 and G3: 72.9 for TGV and 72.5 for TZV, p<0.005 in TGV and p<0.02 in TZV). PSA value increased continuously in 12 men of G1 (57%). Conclusions: These results suggested that the onset of prostate volume increase in Japanese men predominantly occurred at the age of 60 to 75 years old, and volume increase did not necessarily lead to the increase of PSA." @default.
