Approximately 80% to 90% of all causes of thrombosis can now be defined with respect to cause. Of these, more than 50% of all patients harbor a congenital or acquired blood coagulation protein or platelet defect that caused the thrombotic event. It is obviously of major importance to define those individuals harboring such a defect, as this allows: 1) appropriate antithrombotic therapy to decrease risks of recurrence, 2) determination of the time the patient should remain on therapy for secondary prevention, and 3) testing of family members in those harboring a blood coagulation protein or platelet defect that is hereditary (approximately 50% of all coagulation and platelet defects mentioned previously). Aside from mortality, significant additional morbidity occurs from both arterial or venous thrombotic events, including, but not limited to, paralysis (non-fatal thrombotic stroke), cardiac disability (repeated coronary events), loss of vision (retinal vascular thrombosis) and fetal wastage syndrome (placental vascular thrombosis), stasis ulcers, and other manifestations of post-phlebitic syndrome (recurrent deep venous thrombosis [DVT]), etc. A diagnosis of thrombosis is similar to and as generic as a diagnosis of “anemia”; one must, in all instances, as in anemia, ask: What is the etiology of the thrombosis? As in anemia, the specific and appropriate therapy is highly dependent on defining the etiology. The prevalence and mortality of thrombo-occlusive disease versus malignancy in North America is depicted in Table 1 (1). The thrombophilias, in approximate descending probability are depicted in Table 2 (1).
