Webb More articles by this author View All Author Informationhttps://doi.org/10.1016/j.juro.2010.02.2244AboutPDF ToolsAdd to favoritesDownload CitationsTrack CitationsPermissionsReprints ShareFacebookTwitterLinked InEmail INTRODUCTION AND OBJECTIVES This is the study to validate our hypothesis that two photon laser imaging (Robo-Microscopy Project) can be used for imaging the prostate gland architecture and to improve intraoperative differentiation of cancer from benign tissue, thus allowing a more complete cancer removal. METHODS We used tissue sections obtained from the excised prostate. The imaging was carried out using intrinsic fluorescence and scattering properties of the tissues without any exogenous dye or contrast agent. A custom-built imaging tool (Robo-Microscope Project), with a femtosecond pulsed titanium/sapphire laser at 780-nm wavelength was used to excite the tissue; laser power under the objective was modulated via a Pockels cell. Signals were collected at 390 (+/-35 nm), and broadband autofluorescence was collected at 380 to 530 nm. In certain cases, longer wavelength autofluorescence was collected at 530-650 nm. The images obtained from tissue fluorescence were then merged and color coded during post processing for better appreciation of details. The corresponding tissues were subjected to hematoxylin and eosin staining for histological confirmation of the structures. RESULTS On higher magnifications, the acini were clearly distinguishable from the surrounding stroma, and the basement membrane appeared intact. The morphology and arrangement of the cells is roughly the same as in traditional pathology. Figure 1 shows benign cells forming well defined glands where as Figure 2 shows the histopathologic appearance of prostate cancer (B), which lacks individual glands and often fail to form lumens. Robo-Microscopy Project images from the same section also show poorly defined glandular structures (panel A). CONCLUSIONS Our study shows that optical microscopy can assist in differentiating and diagnosing prostate cancer from noncancerous tissues. Further studies will better define its role in actual clinical setting.
