- W2087976092 abstract "Heart failure (HF) is increasing in prevalence among the elderly, especially post-myocardial infarction (MI), in which mortality from HF is higher in the elderly than in the young. We speculate that the aged post-MI population may be at risk for unfavorable renal structural and functional changes which contributes to the mechanism of HF and adverse outcomes. We hypothesize that aged, compared to young rats, will have pre-existing cardiorenal remodeling and dysfunction and that the renal reparative response post-MI will be unfavorable with renal atrophy, Na retention and HF. Studies were performed in young (2 month) and aged (20 month) Fischer rats (n=8/group). MI was produced by left coronary artery ligation (LCAL) while shams had the same procedure without LCAL. At 4 weeks post-MI, cardiorenal structure and function and plasma BNP were assessed. Prior to MI, aged rats compared to young rats were characterized by greater cardiorenal hypertrophy and fibrosis, LV dilatation, reduced GFR and LV dysfunction. At 4 weeks post-MI, the young exhibited cardiorenal hypertrophy and an elevated plasma BNP, with no evidence of HF as Na excretion was preserved. While in the aged, post-MI HF was characterized by a reduction in Na excretion together with worsening LV dilatation and hypertrophy and a failure for plasma BNP to increase. Importantly, MI resulted in a loss of renal mass in the aged.Table 1ParametersYoung-ShamYoung-MIAged-ShamAged-MIHeart Wt:BW(mg/g)2.82±0.043.20±0.05∗2.72±0.06†3.25±0.16∗§LVID(mm)6.46±0.107.43±0.21∗7.20±0.12∗8.34±0.24∗†§LV Fibrosis(%)3.6±0.311.6±1.1∗21.4±1.1∗†22.3±1.0∗†EF(%)85±147±3∗78±2∗†46±3∗§Kidney Wt:BW(mg/g)6.00±0.077.24±0.31∗6.83±0.24∗6.12±0.13†§GFR(ml/min/kg)4.3±0.22.4±0.5∗2.3±0.2∗2.9±0.3∗Urine Na (mEq/day)0.23±0.020.37±0.080.21±0.040.13±0.01†Renal Fibrosis(%)2.8±0.19.1±0.6∗14.0±0.2∗†14.6±0.7∗†Plasma BNP(pg/ml)6±118±4∗9±1†11±2†Mean±SE. ∗P<0.05 vs Young Sham, †P<0.05 vs Young MI, §P<0.05 vs Aged Sham. Aging is characterized by cardiorenal remodeling and dysfunction, which Results in renal atrophy, Na retention and HF post-MI, in contrast to renal hypertrophy and the maintenance of Na excretion in the young post-MI. Further, despite LV dilatation in aged post-MI rats, there was an impaired production and/or release of BNP from the aged heart, which may contribute to the detrimental renal response. Studies are warranted to examine therapeutic strategies to protect the kidney in the aged post-MI population to prevent adverse outcomes." @default.
