- W2088144544 abstract "It was in 1896 that George Beatson first reported benefit from removal of the ovaries in young women with breast cancer’ but it was not until the early 1930s that Taylor first realized the potential of this as a ‘prophylactic’ measure at the time of mastectomy. Numerous retrospective series and non-random comparisons of adjuvant ovarian ablation with ovarian ablation for metastatic disease were reported during the following 30 years. Although they gave conflicting results, the accepted view during this time, which has largely held until the present day. was that it was better to retain ovarian ablation for the treatment of young women with incurable or metastatic disease. However, there were some who saw the need for a more scientific evaluation of adjuvant use and from 1948 to 1965 eight randomized trials were undertaken to assess ovarian ablation as part of primary therapy. Review of the published reports of these trials points to the particular problems associated with conducting trials of ovarian ablation. As well as poor design by today’s standards, slow accrual and non-significant early results hampered acceptance. The first ever randomized trial of cancer therapy was started in 1948 in Manchester by Ralston Paterson. 598 patients, with operable disease and within 2 years of their last menstrual period, were randomized after mastectomy by date of birth for or not for ovarian irradiation. Results showed that although distant relapse was significantly less in the treated group at 15 years (p < 0.05). the mortality difference did not reach significance. However. subgroup analysis showed a survival advantage at 10 years for those entered during the first half of the intake which was not found in the second half. The explanation suggested was that a significantly smaller proportion of the early entrants to the control arm received endocrine therapy on relapse compared to those entered later (p = O.OOl).’ This explanation supported the commonly held view that delaying endocrine therapy" @default.
