- W2088346383 abstract "I T IS CURRENTLY estimated that 35 to 60 million Americans have elevated blood pressure. While all of the secondary forms of hypertension probably account for less than 10% ofthese cases, it is acknowledged that renovascular hypertension is the most common of the forms of curable hypertension. In addition, renovascular hypertension is being recogniz;d as a cause of chronic renal failure more frequently today than ever before. A conservative estimate of the prevalence of renovascular hypertension in our population is 5 % , which would account for 1 to 3 million American hypertensives. Abnormalities of blood vessels, frequently involving the renal artery or its branches, are common in hypertensive patients. However, not all such lesions are causally related to the blood pressure elevation. Renovascular hypertension constitutes the most common remediable form of hypertension and its prevalance is likely to increase in the future. One reason for this anticipated increase is because of the aging of the American population. As individuals become older, atherosclerosis is more frequent and extensive and thus is more likely to cause renovascular hypertension denovo or to superimpose this disorder on a patient with preexisting essential hypertension. In the past, failure to control blood pressure in a hypertensive patient was frequently a reason for that patient to be investigated further for a secondary form of hypertension. However, today, in an era of angiotensinconverting enzyme inhibitors, calcium-channel antagonists, and the increasing array of diuretics, vasodilators, sympatholytic agents, and beta blockers, we can control blood pressure medically in more patients with renovascular hypertension than ever before. For all of these reasons the prevalence and impact of renovascular hypertension is likely to increase. Thus it is important for the practitioner to be familiar with this disease and its manifestations.
