L'hépatite A ne doit plus être considérée comme une affection bénigne de l'enfance. Les progrès de l'hygiène ont raréfié les contacts avec le VHA dans les pays industrialisés, rendant les populations plus réceptives et les épidémies plus massives. Survenant plus tard au cours de la vie, les infections sont plus fréquemment symptomatiques et parfois graves. Le diagnostic de certitude est établi facilement sur la mise en évidence des IgM anti-VHA par immunocapture. Avec l'identification récente du VHE, le diagnostic des hépatites non-A, non-B à transmission entérique n'est plus un diagnostic d'exclusion. La commercialisation des premières trousses ELISA pour la détection des anti-VHE permet de préciser cette étiologie, notamment chez les personnes ayant séjourné dans une région endémique. Dans tous les cas, le diagnostic d'une hépatite virale aiguë devrait résulter d'une démarche économique et rationnelle. Two distinct agents responsible for acute viral hepatitis are transmitted by the fecal-oral route, hepatitis A virus (HAV) and hepatitis E virus (HEV). Hepatitis A should not any more be regarded as a mild child disease. Progress in hygiene practices in the industrialized countries have made rare the VHA exposures, increasing individual susceptibility and epidemic risk. VHA infections, occuring later in the life, are more often symptomatic and sometimes life-threatening. Definitive diagnosis is easy to perform with an anti-HAV IgM immunocapture assay. Since recent identification of HEV, the diagnosis by exclusion of enterically transmitted non-A, non-B viral hepatitis should be specified by the mean of the first commercial ELISA kits for anti-HEV detection, especially among travelers returning from endemic countries. In every case, a cost-saving and rational process should lead to the aetiologic diagnosis of acute viral hepatitis." @default.
