- W2089968562 abstract "V ENOUS THROMBOEMBOLISM has become an increasingly important cause of disability and death during the last 50 yr. There has also been a greater awareness of the ubiquity of the disease and of the possibility that we are experiencing an absolute as well as a relative increase in its occurrence. One of the major difficulties in the management of deep vein thrombosis has been the lack of means for recognizing this condition. Several autopsy studies have shown that, in almost half the patients, there are no clinical signs or symptoms referable to the limbs, and fatal embolism may be the first indication of thrombosis. The urgency of the physician’s concern with venous thrombosis clearly lies in his inability to diagnose the condition accurately at an early stage, when treatment can be effective. During the last few years, several diagnostic tools have become available, including the radioactive fibrinogen test, the ultrasonic technique, impedance plethysmography, and blood coagulability assays. Improvements in the technique of venography have now reached a point where it can be relied upon as a reference standard for evaluating the accuracy of the new methods for detecting thrombi. It is now possible to determine the true incidence of this disease and to study its natural history with some precision. Attempts are being made to recognize individuals or groups of individuals with a “high risk” of developing deep vein thrombosis. It is now possible to evaluate the prophylactic efficacy of a number of choices either available or visible on the horizon, and the accepted methods of treatment are being challenged by several new drugs that are claimed to be significantly superior. There is real hope that the new knowledge arising from the use of improved diagnostic tools will help considerably in establishing a rational approach to the management of deep vein thrombosis.
