Adding to the stress inherent in the problem itself is the relative lack oftherapeutic techniques designed to deal with it. Clinical instructors often find that they havelittle to offer developing therapists other than management strategies intended to bringthe patient through the crisis so that therapy can be resumed. In this article it is argued thatevidence exists to consider the suicidal patient sufficiently different from other patients towarrant development of a distinct treatment. Evidence indicating the uniqueness of suicidalpatients on a variety of cognitive dimensions is reviewed and possible directions for cognitiveintervention and future research outlined.Recent years have seen psychotherapy move steadily away fromunitary theories of etiology and treatment toward more problem-specific models (e.g., Beutler, 1979; Goldstein & Stein, 1976;Lazarus, 1984c). Far from assuming, for example, that all emo-tional and behavioral problems are rooted in unconscious conflictsresolvable only through years of uncovering therapy, we now findthat some disorders are more responsive to some therapies thanto others. Thus, we find simple phobias to be especially responsiveto systematic desensitization (Walen, Hauserman, & Lavin, 1977;Wolpe, 1982), we see behavioral techniques being adapted spe-cifically for use with sexual dysfunctions (e.g., Kaplan, 1974),and we learn that programs specifically designed for depressionrival pharmacological treatments in effectiveness (Rush, Beck,Kovacs, & Hollon, 1977; Weissman, Klerman, Prusoff, Sholom-skas, & Padian, 1981). Several authors (Goldstein & Stein, 1976;Halleck, 1978; Lazarus, 1984b; Strupp, 1978) have expressed theneed for more knowledge about which approaches work best withwhich patients and under which circumstances.The issue addressed in this article is whether a case can bemade for extending the principle of specialized therapy to thesuicidal patient. Specifically, the question is, Have studies of cog-nitive characteristics of the suicidal individual sufficiently differ-entiated the suicidal patient from other patients to warrant a distincttreatment strategy? An attempt is made to answer this question inthe affirmative by reviewing the research in the area and present-ing the findings as evidence for the potential utility of cognitivetherapeutic interventions with suicidal patients." @default.
