- W2090297922 abstract "To evaluate whether oral estradiol (E2) without gonadotropin-releasing hormone (GnRH) mediated down regulation can adequately suppress luteinizing hormone (LH) and prepare the endometrial lining in patients undergoing transfer of thawed blastocysts immediately following a previously failed in vitro fertilization (IVF) cycle. Retrospective analysis. In an attempt to expedite the transfer of frozen embryos, patients were treated with oral E2 for endometrial preparation on the 1st menses following a cancelled or failed IVF cycle. Subjects were stratified by the outcome of their preceding fresh or frozen cycle and outcomes in the following cryosynthetic (CS) cycle using E2 alone without GnRH. Chi-square test was used to analyze age, interval of days between cycles, progesterone (P4) level and endometrial thickness (ET) at time of progesterone initiation within each CS cycle outcome group. Patients (n=73) were treated with E2 without GnRH suppression. The preceding IVF cycle ended in either cancellation due to ovarian hyperstimulation (4.2%), failure to implant (73.6%), or biochemical pregnancy (22.2%). Estrogen was begun to prepare the endometrium 2 days following the onset of the first menses after a negative hCG. At time of P4 initiation, all subjects had P4<1.3 ng/ml and ET>8mm (60.3% = Type 1). E2-only CS cycles achieved a 45.8% successful pregnancy rate (SPR). Time waited in between cycles was not correlated with cycle outcome (P=0.9). The endometrium, at P4 initiation, was significantly thicker in CS cycles achieving a SPR (8.72+/-0.32mm) compared with cycles ending in failure (8.53+/-0.45mm) or biochemical pregnancy (8.36+/-0.68mm)(P<0.05). It is not necessary to wait a month after a failed IVF cycle, nor is it necessary to down-regulate these patients with a GnRH agonist. Initiating CS cycles with oral E2 adequately suppresses pituitary LH and facilitates endometrial preparation and embryo implantation without an increased risk for premature luteinization." @default.
