- W2090576576 abstract "To establish a new method for total body irradiation (TBI) with a field-in-field technique using jaws. Patients received TBI with 10 MV X-rays from a linear accelerator (linac) equipped with asymmetric jaws. An extended source-to-axis distance of 470 cm was used to provide a single large field. Patients were treated in a supine position with a pair of opposing horizontal beams and 2 more pairs of opposing horizontal beams were added to improve the dose homogeneity. In the first field (whole body field), the monitor unit (MU) required to deliver a prescription dose at the mid-plane of the head region was determined. In the second field (trunk field), the MU to deliver a dose compensating for the difference of the width of the head and the trunk was calculated. In the third field (pelvis field), the MU to deliver a dose compensating for the difference of the width of the trunk and the pelvis was used. The jaw opening for each field was determined based on the whole body CT of the patient. We performed the measurements for output factor (OPF), percent depth dose (PDD), and off-center ratio (OCR). A Farmer-type chamber was used to measure dose and was inserted in a water-equivalent solid phantom. Two water phantoms were placed on both sides of the solid phantom to take into account the effect of side scatter in the large field. The jaw opening in the anterior-posterior direction was 70.5 cm in the detector plane. The jaw openings in the cephalo-caudal direction for the 3 fields were 188, 94, and 47 cm, respectively. In vivo dosimetry was performed using fluoroglass dosimeters with a RANDO phantom. The dose difference in the head region of the RANDO phantom was within 6.4%, the cervical region was within 5%, the trunk region was within 2.8% and the pelvis region was within 2% of the prescribed dose. The dose difference in the lung region was 11% larger than the prescribed dose. However, the dose of the lung region can be decreased using MLC. The field-in-field technique does not require special apparatus, but can improve dose homogeneity in TBI. Asymmetric jaws are usually available in modern linear accelerators. Therefore, this technique can be performed in almost all institutions." @default.
