- W2091210115 abstract "Skin desensitization by topical anesthetic was studied for its effect on the frequency and amplitude of 3 overt involuntary oscillations of the ankle: pathological clonus, physiological clonus, and physiological action tremor. Pathological clonus was studied in two spinal cord patients, physiological clonus in 3 normal subjects following prolonged exercise, and physiological action tremor in 7 normal subjects performing voluntary movements. Topical anesthetic applied to the skin of the leg, ankle and proximal half of the foot resulted in a significant decrease in the amplitude of all 3 oscillations with no significant change in frequency. The effect of topical anesthetic on pathological clonus amplitude was so profound that neither patient could initiate3clonus for approximately 30 min, despite repeated attempts to do so. While the mechanism determining the frequencies of these oscillations is a matter of considerable debate, the consistency in their response to topical anesthetic suggests that a common mechanism may exist. A possible clinical application is suggested. Topical anesthetic may be a non-invasive means of attenuating undesirable large amplitude tremors and clonus. La désensibilisation de la peau par anesthésie locale a été étudiée pour ses effets sur la fréquence et l'amplitude de 3 types d'oscillations involontaires claires de la cheville, clonie pathologique, clonie physiologique et tremblement physiologique actif. La clonie pathologique a étéétudiée chez deux patients spinaux, la clonie physiologique, chez 3 sujets normaux après un exercice prolongé et le tremblement physiologique actif chez 7 sujets normaux exécutant des mouvements volontaires. L'anesthésiant local appliqué sur la peau de la jambe, de la cheville et sur près de la moitié du pied a entraîné une diminution significative de l'amplitude des 3 types d'oscillations sans modification significative de la fréquence. L'effet de l'anesthésie locale sur la clonie pathologique a été si spectaculaire qu'aucun des patients n'a pu présenté un état de clonies pendant environ 30 min en dépit de tentatives répétées. Alors que les mécanismes déterminant les fréquences de ces oscillations reste l'objet d'un important débat, la stabilité de leur réaction à l'anesthésie locale suggère l'existence éventuelle d'un mécanisme commun. Une possible application clinique est suggérée. L'anesthésie locale pourrait être un moyen non-invasif d'atténuer de façon importante les grandes amplitudes de clonies ou de tremblements génants." @default.
