- W2091330677 abstract "To evaluate the value of umbilical artery Doppler study as a predictive marker of perinatal outcome in preterm pregnancies with small for gestational age. Total of 218 patients were at 27–36 weeks of gestational age who received antenatal umbilical artery Doppler velocimetry and delivered singleton small-for-gestational age infants. The ratio of peak-systolic to end-diastolic (S/D) blood flow velocities in the umbilical artery was measured in each patient. The patients were divided into three groups: the normal group with S/D ratios of less than 95th percentile (n = 134), elevated S/D ratio group of more than 95th percentile (n = 41), and those with absent or reversed S/D ratios (n = 43). These groups were comparatively analyzed with respect to maternal characteristics and neonatal outcome. In each group, the gestational age at the time of diagnosis of SGA, the mean gestational age at delivery, and the mean birthweight showed statistically significant difference (P < 0.001). The time interval from diagnosis to delivery showed significant difference (P = 0.002). In addition, the incidence of low Apgar scores of less than 7 at 5 minutes, duration of stay in the neonatal intensive care unit, duration of ventilator care, perinatal death, and poor perinatal outcome were significantly increased in infants with abnormal S/D ratio (P < 0.001). When logistic regression analysis was performed after adjusting for gestational age, the odds ratio for poor perinatal outcome was 3.7 in the group showing abnormal S/D ratio (95% confidence interval 1.42–9.54, P = 0.007). Umbilical artery Doppler velocimetry is shown as a significantly efficient method in predicting perinatal outcome in preterm neonates with small-for-gestational age, and it may be useful in managing preterm patients with small-for-gestational age fetuses." @default.
