- W2091492983 abstract "小児ネフローゼ症候群の頻回再発例 (FR) にcyclophosphamide (C) を投与する場合，C開始前にステロイド剤を増量し完全寛解に導入しなくても，再発抑制効果が変わらないかを検討した。FR32例を対象とし，再発時点でprednisolone (PSL) を増量し，完全寛解に導入した後にCを開始したA群 (15例) と，再発時点でPSLを増量せず，Cを開始したB群 (17例) に分け，B群のうち寛解に至ったものをB'群 (14例) として以下の結果を得た。1) A群，B'群は同様のclinical profileだった。2) C投与前後で再発頻度は有意に減少し (A群; 投与前2.3±0.9，後0.6±1，B'群; 前2.7±1.2，後0.4±0.5回/年，いずれの群も投与前後でP<0.01)，また，A群とB'群の再発頻度には有意差はなかった。3) 再発時点から1年間の平均PSL総投与量はB'群ではA群に比し有意に少なかった (A群; 141±61，B'群; 80±57mg/kg/年，P<0.01)。4) Cの副作用は両群ともに各2例 (A群13%，B'群14%) に排尿時痛が見られたが一過性であった。結論として，FRにCを投与する場合，開始前に完全寛解に導入することに明らかな有用性は認められず，再発した時点でステロイド剤を増量することなくC投与が可能である。" @default.
