• W2092272920 abstract "Necrotizing enterocoltis (NEC) is a life threatening complication during the treatment of acute leukemia (AL). of our investigation is to analyze the cases of NEC in patients with AL, to choose the main clinical and laboratory criteria and to propose the best treatment. 6 cases of AL complicated with NEC were analyzed. The group included 5 boys and a girl aged from 5 to 15 years, 3 of them suffered from ALL and 3 suffered from AML. All patients were treated with appropriate chemotherapy regimens: ALL IC-BFM-2002 or AML-BFM-2003. The plan of examinations included: blood count, abdominal ultrasound, stool culture. 83% of assessed patients were males with mean age of 11 years. The main clinical symptoms of NEC were: fever >38°C, abdominal pain in right-lower-quadrant, vomiting, diarrhea. The main laboratory change was profound neutropenia. The main ultrasound sign was thickening of bowel wall >5 mm. Stool cultures found different types of microorganisms. Treatment algorithm included: combination of broad-spectrum antibiotics, antifungal coverage, i.v. Immunoglobulin and G-CSF administration, total parenteral nutrition. The mainstay of NEC management in patients with AL is conservative treatment. Surgical intervention should be reserved for severe complications, such as appendicitis, bowel perforation, peritonitis. Nekrotyczne zapalenie jelita grubego jest groźnym powikłaniem zakaźnym ostrej białaczki. Wyodrębnienie zasadniczych kryterów diagnostycznych i propozycja postępowania leczniczego w nekrotycznym zapaleniu jelita grubego u dzieci z ostrą białaczką. Poddano analizie 6 przypadków ostrej białaczki, powikłanej nekrotycznym zapaleniem jelita grubego u 5 chłopców i 1 dziewczynki w wieku 5–15 lat. U 3 dzieci rozpoznano ostrą białaczkę limfoblastyczną (ALL), u 3 – mieloblastyczną (AML). U wszystkich pacjentów stosowano chemioterapię (w ALL – ALLIC-BFM-2002; w AML – BFM2003). Stosowane badania: morfologia krwi, ultrasonografia brzucha, bakteriologiczne badania krwi i stolca. Objawami klinicznymi nekrotycznego zapalenia jelita grubego są: gorączka, bóle brzucha, wymioty, biegunki. We krwi obwodowej zawsze występowała neutropenia <0,5 × 109/L. Sonograficznie naciek błony śluzowej kątnicy od 5 do 26 mm. Bakteriologiczne badania stolca ujawniały rozmaite bodźce bakteryjne i grzybicze. Leczenie polegało na stosowaniu kombinacji antybiotyków o szerokim zakresie działania, środków przeciwgrzybiczych, dożylnej immunoglobuliny, czynników stymulujących kolonie granulocytarne, pozajelitowego żywienia. W leczeniu nekrotycznego zapalenia jelita grubego u dzieci z ostrą białaczką niezbędne są: 1. antybiotyki o szerokim zakresie działania, 2. dożylne środki przeciwgrzybicze, 3. czynniki stymulujące kolonie granulocytarne, 4. dożylne immunoglobuliny cotygodniowo, 5. żywienie pozajelitowe, 6. codzienne monitorowanie w celu wczesnego rozpoznania perforacji jelita grubego i leczenia chirurgicznego." @default.
