- W2092561657 abstract "Thirty-five patients with symptoms or signs of diabetic neuropathy were tested for autonomic neuropathy by measuring heart rate and blood pressure changes during an orthostatic tilt test, a single deep breath and the Valsalva manoeuvre and these results were related to electrophysiological measurements made on the common peroneal and sural nerves. The sural sensory action potential (SAP) was more strongly correlated with these tests of autonomic function (particularly with the brake index of the orthostatic tilt test (P < 0.001) and the fall in systolic blood pressure 1 min after tilt (P < 0.001), than the common peroneal nerve compound motor action potential, its minimum F wave latency or nerve conduction velocities. Patients with a detectable sural SAP had significantly higher brake indices than those with absent sural SAPs. Significant correlations were also obtained with the common peroneal motor nerve conduction velocity (MNCV) and autonomic tests and patients with MNCV 〈and〉 38 m/sec showed significant differences in many autonomic tests. The sural SAP amplitude, being less susceptible to factors that influence nerve conduction velocity, may be useful in identifying patients with an autonomic neuropathy. Trente-cinq patients présentant des symptômes ou des signes de neuropathie diabétique ont étéétudiés dans l'hypothèse d'une neuropathie autonome en mesurant les modifications du rythme cardiaque et de la pression sanguine pendant un test d'inclinaison orthostatique, une inspiration profonde unique et une manoeuvre Valsalva; ces résultats ont été corrélés avec les mesures électrophysiologiques faites sur les nerfs péronéal commun et sural. Le potentiel d'action sensoriel du sural (PAS) était plus fortement corrélé avec ces tests de fonction autonome (en particulier avec l'indice de freinage du test d'inclinaison orthostatique (P < 0,001) et la chute de la pression sanguine systolique 1 min après l'inclinaison (P < 0,001), que le potentiel d'action moteur composé du nerf péronéal commun, la latence minimale de l'onde F ou les vitesses de conduction nerveuse. Les patients avec un PAS sural détectable avaient des indices de freinage significativement plus grands que ceux pour lesquels le PAS sural était absent. Des corrélations significatives ont aussi été obtenues avec la vitesse de conduction du nerf moteur péronéal commun (VCNM) et les tests autonomes et les patients avec VCNM 〈et〉 38 m/sec montraient des différences significatives dans de nombreux tests autonomes. L'amplitude du PAS sural, étant moins assujettie aux facteurs qui influencent la vitesse de conduction nerveuse, peut être utile pour identifier les patients atteints d'une neuropathie autonome." @default.
