- W2093640641 abstract "The possibility that cognitive stimulation can have a positive effect in postponing the incidence of dementia or slowing the rate of progression of the disease has been tested several times with discordant results. Presence of important sensory deficits can play a role in preventing the positive effect of this stimulation. Few studies explored this association in very old people, where prevalence of auditory and visual impairments is very high. To study the association between auditory or visual deficits and incident dementia and rate of progression of the disease in the very old (80 years-old and over). Of the 1492 subjects who received a baseline interview at home or at institution by trained psychologist and were still alive one year later, 1450 (97%) had at least one follow-up interview: 995 were dementia-free and 455 were demented at baseline. Diagnosis of dementia was based on DSM-IV criteria. Presence of a serious sensory deficit was defined as a deficit severe enough so that it could interfere with the administration of the neuropsychological tests. The cross-sectional analysis showed a highly statistically significant association between presence of sensory deficits and dementia: 21% vs. 10% at the univariate analysis (p<0.0001), which remained significant after correcting for age, sex and education. On the contrary people who developed dementia had not a higher presence of sensory deficits relative to people who remained non-demented (35% vs. 30%, p=0.38). In the population demented at baseline a slight but statistically significant difference in the rate of change of MMSE score was visible in subjects with auditory problems (yearly rate of change of people with this impairment was 1.00 MMSE point higher than their counterpart, p=0.039); this result remained significant (p=0.049) also when corrected for age, sex and education. In the very old, while presence of auditory or visual problems does not seem to interfere with incidence of dementia, auditory problems may have an effect on the rate of progression of dementia." @default.
