Ou à l'inverse faut-il proposer une éviction totale du sujet par mutation, voire exclusion professionnelle? Une approche est proposée aussi bien pour un asthme que pour une alvéolite. Enfin le cas des adolescents et des adultes atopiques, symptomatiques ou non, souhaitant travailler dans des métiers dits à risques, exige une décision au cas par cas. Elle ne saurait justifier une exclusion a priori, même pour des expositions à des substances de haut poids moléculaire. Prevention is required in various situations based on the concept that occupational respiratory allergy is not a fatality. For asymptomatic subjects exposed to asthma-inducing substances, prevention is primarily designed to modify working conditions by decreasing or eliminating the risk: change of molecules or transformation of the process, capture and aspiration of pollutants at the source, completed when necessary by the wearing of individual protection. Management of the triggering factors published in the literature is an invaluable aid. There is no demonstrated basis for tests to common respiratory allergens prior to employment. A different strategy must be adopted in the case of epidemic or sporadic cases : should the subject be maintained at his job while drastically modifying the exposure ? Or, on the contrary, should complete elimination of exposure be proposed by changing the subject's job or even excluding him from his work ? An approach is proposed for asthma and for alveolitis. Finally, the case of symptomatic or asymptomatic atopic adolescents and adults wishing to work in so-called high-risk jobs requires a case-by-case decision. Systematic exclusion cannot be justified, even in the case of exposure to high molecular weight substances." @default.
