- W2094637963 abstract "Patients with Lewy body disease (LBD) often have a combination of Alzheimer's disease (AD) and LBD pathologies at autopsy. Since hippocampal atrophy is associated with the neurofibrillary pathology of AD, hippocampal volume measurements may be useful in determining the severity of AD pathology in patients with LBD. Our objective was to determine whether antemortem hippocampal atrophy on MRI is associated with AD pathology in pathologically proven cases of LBD. We identified 116 subjects who underwent antemortem MRI and had AD and/or LBD type pathology ranging from low to high likelihood AD and LBD at autopsy. Subjects were pathologically classified as low likelihood of AD/no LBD (normal) (n = 27), high likelihood of AD/no LBD (n = 33), high likelihood of LBD (n = 27), intermediate likelihood of LBD (n = 18), and low likelihood of LBD (n = 11) according to the NIA-Reagan Criteria for AD and the 3rd Consortium Criteria for LBD. Hippocampal volumes were estimated from 3D-volumetric MRI with an automated segmentation algorithm on SPM5 and scaled to the total intracranial volume. We investigated the associations between hippocampal volumes and the classification of AD and LBD pathologies and Braak neurofibrillary tangle (NFT) stage adjusting for age, gender and antemortem MRI to death interval on statistical analysis. Antemortem hippocampal volumes were smaller in patients with a high likelihood of AD, low likelihood of LBD and intermediate likelihood of LBD compared to the patients with a high likelihood of LBD and low likelihood of AD/no LBD (normal) (p < 0.01). Smaller antemortem hippocampal volumes were associated with higher Braak NFT stage in this cohort of normal, LBD, mixed AD/LBD and AD cases (r = −0.59; p < 0.001). Hippocampal volumes distinguished cases with low likelihood of AD from intermediate or high likelihood of AD with an area under the curve of 0.84 independent of the presence of LBD pathology." @default.
