- W2094837983 abstract "We report two cases of strumal carcinoid (SC) of the ovary occurring in 14 and 58 year old females. Both patients underwent salpingo-oophorectomy for multilocular cystic ovarian tumours detected by CT. Macroscopically, the tumours had yellowish or brown cystic and solid lesions and evidence of a mature cystic teratoma was found in the first case. Microscopically, a proliferation of macro/micro-follicular thyroid-like tissue was seen, with mild cellular atypia and arranged partially in a trabecular and partially in an insular or cord-like growth pattern, reminiscent of carcinoid tumours. In addition, both cases contained mature epidermis and skin appendages. Immunohistochemically, carcinoid elements were diffusely positive for chromogranin A (CGA), synaptophysin, CD56 and several polypeptide neuroendocrine hormones, and focally positive for calcitonin. However, follicular epithelium was only positive for thyroglobulin and thyroid transcription factor-1 (TTF-1). Interestingly, double immunostaining revealed the distribution of a small number of both TTF-1- and CGA- or calcitonin-positive cells. These features are consistent with SCs, which are considered as benign neoplasms. As teratomatous elements and double-immunostained cells were found, it is likely that SCs originate from germ cells, namely pluripotential stem cells capable of differentiating multidirectionally. However, this remains an enigma. Se presentan dos casos de carcinoide estrumal (CE) del ovario en mujeres de 14 y 58 años. El estudio radiológico con TAC mostró tumores quísticos, multiloculares de ovario en las dos pacientes, realizándose una salpingooforectomia en ambos casos. El examen macroscópico reveló tumores de color amarillento o marrón con lesiones quísticas o sólidas. En el primer caso se encontraron otros elementos de teratoma quístico maduro. El examen microscópico mostró una proliferación de tejido tiroideo macro/microfolicular con atipia celular moderada, mezclado con un patrón de crecimiento parcialmente trabecular y parcialmente en cordones o insular, semejante al de los tumores carcinoides. Además, en ambos casos existían epidermis madura y anejos cutáneos. En el estudio inmunohistoquímico los elementos carcinoides fueron difusamente positivos para cromogranina A (CGA), sinaptofisina, CD56 y varias hormonas neuroendocrinas y, focalmente, para calcitonina. El epitelio folicular fue sólo positivo para tiroglobulina y TTF-1. La doble inmunotinción mostró un pequeño número de células positivas simultáneamente para TTF-1, CGA y calcitonina. Todo ello configura un diagnóstico de carcinoide estrumal, un tumor benigno. Debido a la coexistencia de áreas teratomatosas y células con doble tinción, es posible que su histogénesis sea a expensas de células germinales, es decir, células madre pluripotenciales capaces de diferenciación múltiple.
