- W2095263145 abstract "Abstract The various aspects of diverticulitis, based on a review of the literature and our clinical experience, have been presented. It is apparent that some phases of this disease entity are still controversial and warrant further investigation and follow-up studies. However, it is generally conceded that uncomplicated acute diverticulitis should be treated medically, while the complication of diverticulitis (e.g., perforation, abscess, obstruction and fistulas) demands surgical intervention. When carcinoma cannot be ruled out, resection of the involved segment is indicated. Chronic or recurring diverticulitis should be treated similarly in our estimation. One hundred seventeen cases of sigmoidal diverticulitis were treated by the authors. Of this number forty-one were submitted to surgery. Resection was performed in twenty-nine cases with one death, a mortality rate of 3.4 per cent. Of the major surgery performed, six were by the exteriorization method of Mikulicz, four by abdominoperineal proctosigmoidectomy, one by abdominoperineal proctosigmoidectomy and hemicolectomy, and twelve by resection followed with end-to-end anastomosis. In six others sigmoidectomy with permanent colostomy and extraperitonealization of the closed rectal stump by the Hartman technic was instituted. Partial cystectomy was required in four instances. It is our opinion that prior to surgical intervention each patient must be carefully individualized, particularly in relation to the procedure best suited to his or her case." @default.
