In the fatal case described, the tamponade resulted from perforation of the superior vena cava. The pericardial cavity has cephalad boundaries that reach the level of the aortic arch and iunominate vein. Hence perforation of the superior vena eava below the pericardial reflection is a feasible mechanism of inducing tamponade as was demonstrated at autopsy. The authors suggest avoidance of the left internal jugular vein approach and using catheters made of less rigid material, such as Silastic and polyurethane. They emphasize the importance of knowing the position of the catheter tip, which can lie in the proximal superior vena cava above the reflection of the pericardial cavity (ie, above the level of the aortic knob), as CVP at any site in the great veins is within 1 m m Hg of that of the right atrium.
