- W2099040080 abstract "The advanced cardiac life support (ACLS) course was developed by the American heart association (AHA) in the 1970′ to provide a series of standard treatment guidelines and procedures, which is supposed to be one of the foundations in the management of cardiac arrest (CA) and sudden cardiac death.1Field J. Hazinski M. Sayre M. et al.Part 1: Executive summary. 2010 American heart association guidelines for cardiopulmonary resuscitation and emergency cardiovascular care.Circulation. 2010; 122: S640-S656Crossref PubMed Scopus (771) Google Scholar Studies have shown that new interns or physicians have poor life support competencies.2Price C. Cardiopulmonary resuscitation training, knowledge and attitudes of newly-qualified doctors in New Zealand in 2003.Resuscitation. 2006; 68: 295-299Abstract Full Text Full Text PDF PubMed Scopus (26) Google Scholar According to a survey of graduating 4th-year medical students, 36.8% students avoided participating in resuscitation due to lack of training, and 91.7% students believed ACLS should be one part of medical student curriculum.3Pillow M.T. Stader D. Nguyen M. et al.Perceptions of basic, advanced, and pediatric life support training in a United States medical school.J Emerg Med. 2014; 46: 695-700Abstract Full Text Full Text PDF PubMed Scopus (14) Google Scholar In the vast majority of American hospitals, there are ACLS-trained medical staffs, and it is mandatory for all emergency medical staffs to receive ACLS training.4Filgueiras Filho N.M. Bandeira A.C. Delmondes T. et al.Assessment of the general knowledge of emergency physicians from the hospitals of the city of Salvador (Brazil) on the care of cardiac arrest patients.Arq Bras Cardiol. 2006; 87: 634-640Crossref PubMed Scopus (9) Google Scholar However, in China, very few hospitals required all resident physicians to receive ACLS training.5Zhang Y. Cheng L. He C. et al.Training program of ACLS among medical staffs.J Pract Nurs. 2001; 17 ([in Chinese]): 46Google Scholar And in most Chinese medical schools, ACLS training is not a regular part of the clinical curriculum. We conducted an assessment on fresh medical staffs using the ACLS pre-test paper (2012 version) on September 2013. The paper contained 3 sections, namely electrocardiograph (ECG) (10 questions), pharmacology (10 questions), and practical application (20 questions). The ECG section tested the ability to identify the most common normal and abnormal ECGs, which include ventricular fibrillation, ventricular tachycardia, etc. The pharmacology section tested when and how to use medications correctly in those complicated ACLS scenarios, and the practical application section asked students to identify rhythms and choose correct interventions or drugs. A total of 362 participants who just graduated from a variety of medical schools in different Chinese provinces took part in the assessment, and 346 (95.58%) evaluations were valid. Of whom, 92.20% (N = 319) were male, 97.07% (N = 331) held a bachelor's degree, while the rest being either a Master (2.89%, n = 10) or a Doctor (1.45%, n = 5). The average age was 24.80 ± 1.25 years old. Averagely 20.12 ± 4.13 out of 40 questions were correct, ranging from 6 to 31. The average accuracy rate for ECG, pharmacology and practical application section was 49.68%, 51.39% and 50.08% respectively, which contributed to the total average accuracy rate of 50.31%. Ten questions contributed the accuracy rates of no more than 30%, ranging from 6.07% to 28.32%. Of these, 7 questions were from practical application section, 2 from ECG section and 1 from pharmacology section. Our assessment indicated that fresh Chinese medical staffs have poor knowledge of ACLS, even though they have received five years’ medical study. However it is very important for fresh medical staffs to have ACLS competencies, with which they may rapidly be accustomed to clinical work and be able to do good team work with their colleagues to deal with the critical ill patients. We therefore suggest a specific course should be developed in medical schools, focusing on building a good foundation for medical students to provide best life support in their future vocational life. The study was supported by the Innovation Program of Shanghai Municipal Education Commission (Grant No. 12ZS080). The authors thank all the medical students for their cooperation." @default.
