- W2099241838 abstract "A 54-year-old man with a history of hypertension presented with sudden onset of palpitation, chest pain, and dizziness. Frequent runs of nonsustained broad complex tachycardia were captured on ambulance 3-lead electrocardiogram with an inferior axis, suggestive of outflow tract ventricular tachycardia (VT). VT resolved spontaneously on admission at the coronary care unit. Subsequent echocardiography revealed normal biventricular function with normal sized left and right ventricles and no significant valvular abnormality. Electrolytes including serum potassium and magnesium levels were within normal limits and cardiac enzymes were not raised. An exercise tolerance test was unremarkable, and the patient already had a normal coronary angiogram 2 months earlier as part of investigations for chest pain. The overall clinical impression was therefore normal heart idiopathic outflow tract VT. The patient had incessant runs of nonsustained and sustained VT the next morning, which was associated with intermittent hemodynamic compromise, as well as 12-lead electrocardiographic features of right ventricular outflow tract (RVOT) VT (left bundle branch block morphology and inferior axis; Figure 1). He was given an intravenous bolus of 2 g of magnesium sulfate, which resulted in the complete resolution of VT.
