- W2100023076 abstract "Complex I deficiency (CID) is a disorder of oxidative phosphorylation, characterized by impaired mitochondrial ability to oxidize NADH. Triglyceride as compared to glucose oxidation provides a higher non-NADH vs. NADH electron flow to the respiratory chain; we thus postulated that supplying more fat could be benificial for CID patients. We studied the effects of intravenous infusion of Intralipid (1.83.6 mg/kg/min triglyceride) or glucose (510 mg/kg/min) in a patient with primary CID presenting with myopathy and severe exercise intolerance, at restduring exercise, with indirect calorimetry, 31P Nuclear Magnetic Resonance Spectroscopy (NMRS), and bicycle ergometry. NMRS in forearm muscle during steady-state submaximal exercise (bulb squeezing) showed improved mitochondrial performance (i.e. lower inorganic phosphate/phosphocreatine ratios (mean ±SD: 0.40 ±0.14 vs. 0.71 ±0.06, P = 0.001)), and 1.5 times higher maximal power output, during fat compared to glucose infusion, respectively. In leg muscle, stationary bicycling at 7 Watt was endured 1.5 times longer with fat compared to glucose infusion. Oxygen consumption during cycling was significantly higher during fat infusion compared to glucose infusion (368 ±26 vs. 326 ±23 ml/min, P < 0.001). Fat oxidation provided markedly more energy during fat vs. glucose infusion (difference: at rest 14 energy%, and during cycling 29 energy%, P < 0.005). We conclude that in CID, fat as compared to carbohydrate improves mitochondrial performance of oxidative phosphorylation during exercise by skeletal muscle. The findings support the preferential use of fat for dietary management of CID patients." @default.
