(1) related with the adherence to lipid-lowering agents. Although optimal drug adherence was found to be related to the presence of co-morbidities, follow-up visits and attendance in family medicine specialist clinics, younger patients, patients without co-morbidities, new patients and patients visiting clinics other than family medicine specialist clinics were found to be non-compliant. Although statins have well-established benefits in the primary and secondary prevention of coronary heart disease and stroke, and inadequate management of lipids is associated with adverse outcomes, suboptimal adherence to lipid- lowering medications is common (2, 3). However, the reasons for poor adherence are not well understood. In a recent study (4), in patients with coronary heart disease, a 26% rate of non-compliance to statins was found, which was in turn associated with an 85% increase in overall mortality. Although the causes for non-compliance are multifactorial, the lack of education and awareness about long-term benefits of treatment may be one of the major reasons. In a recent study of Yilmaz et al. (5), the authors evaluated the level of awareness of patients for the prescribed statins, and demonstrated that 16.5% of 236 participants had no idea about their treatment; however, 58% of patient did not know how long they would have to use their statins. In a recent study of Schneeweiss et al. (6), it was found that full out-of-pocket spending for statin prescription nearly doubles the likelihood of stopping treatment. On the other hand similar to the findings of the study of Yilmaz et al. (5), in a recent study (7), it was demonstrated that patients were more likely to comply if the statin was prescribed by a cardiologist or a primary care doctor. In another study by Yilmaz et al. (8), it was demonstrated that providing patients with comprehensive knowledge about statins, improve adherence and also increase the percentage of patients reaching targets. The unfounded fear of patients about the toxicity of statins may play an important role in non-compliance. On the other hand, in these recent studies (1, 5), it was demonstrated that the presence of co-morbidities or co-medications seem to increase the adherence to statin therapy. In conclusion, dyslipidemia is one of the major cardiovascular risk factors, and lipid-lowering therapy primarily with statins is the mainstay in primary and secondary prevention of cardiovascular disease and stroke. However, overall compliance to statin therapy is still suboptimal. Considering the long-term adverse effects of poor adherence, interventions that can improve long-term treatment compliance should be encouraged. None." @default.
