- W2101636665 abstract "Surgery has been the mainstay of treatment for appendicitis since Charles McBurney first demonstrated successful surgical management in the late 1880s. Since that time, there has been a paucity of research looking at other ways of treating appendicitis, and it was not until the development of powerful antibiotics that other viable methods existed. In this meta-analysis, the authors analyzed six papers that compared antibiotic management to surgical management, specifically looking at the need for surgery after antibiotics, complication rates, and re-infection rates. Studies were included if they compared surgery to antibiotics in low-risk patients without a known abscess. Exclusion criteria included unknown antibiotic regiments, inclusion of patients with symptoms for more than 3 days, known pre-existing abscess, and studies that included only pediatric patient populations. Six reports with 1201 patients were included in the final analysis. Of 433 patients who were treated with antibiotics alone, the overall failure rate was 6.9±4.4%. Of those patients who were initially successfully treated with antibiotics, 14.2±10.6% had a recurrence of appendicitis within 1 year of treatment. When excluding the patients who required an eventual appendectomy, complication rates were significantly more common in patients undergoing surgery (antibiotics 3.5±8.5% vs. surgery 13.1±10.7%).
