- W2102709618 abstract "Physical activity has been shown to have beneficial effects in several chronic diseases and has been suggested to reduce therisk of cognitive decline and dementia in the elderly. Walking is the most common and viable physical activity, especially in the very old. The possible benefits associated with walking on incidence of dementia has been explored in few studies and no evidence is available on this association in the oldest olds. To examine the relationship between walking and incidence of dementia in a prospective, population-based study of eighty years or older residents in eight municipalities of Varese province, Italy (the Monzino 80-plus Study). Of the 1584 survivors of the baseline population (n = 1842), 1412 elderly (89.1%) were re-interviewed at first follow-up. Diagnosis of dementia was based on DSM-IV criteria. Information on leisure-time physical activities were collected and participants were asked to estimate the average time per day spent walking. Reporting to walk less than an hour a day was strongly associated at baseline with dementia in univariate (p < 0.0001) and multivariable analyses (p = 0.0092). Among the 896 non-demented elderly at baseline (mean age 86.9 ± 4.3 years, 72% women, mean education 5.4 ± 2.6 years), after an average 1.6 years, incidence of dementia was higher (p = 0.0001) among the elderly who walked less than an hour a day (21.3%) than those who walked one hour or more a day (11.8%). The risk of developing dementia remained significantly increased among the elderly who walked less than an hour a day also in a multivariable model (OR, 1.7; 95% CI, 1.1–2.5) where age, sex, education, diabetes, hypertension, heart failure, chronic obstructive pulmonary disease, ictus, Parkinson's disease, depression, and duration of follow up were entered as covariates. This association persisted also when baseline MMSE score, leisure activities, social activities, and economic status were added to the previous model (OR, 1.6; 95% CI, 1.1–2.5).
