- W2105602285 abstract "We read with interest the case by Fields et al1Fields J.M. Chandra S. Au A.K. et al.Images in emergency medicine. Child with painful palmar mass. Superficial palmar arch pseudoaneurysm.Ann Emerg Med. 2013; 62: 569Abstract Full Text Full Text PDF PubMed Scopus (6) Google Scholar on a posttraumatic pseudoaneurysm of the superficial palmar arch that was successfully treated conservatively. We first advocated this treatment option in 2003 for a similar lesion affecting the radial artery of a 3-year-old girl.2Cozzi D.A. Morini F. Casati A. et al.Radial artery pseudoaneurysm successfully treated by compression bandage.Arch Dis Child. 2003; 88: 165-166Crossref PubMed Scopus (27) Google Scholar We recently encountered another case, a pseudoaneurysm of the radial artery, which occurred in a 10-week-old hemophilic infant after arterial puncture.3Ceccanti S. Frediani S. Andreoli G.M. et al.Effective compression bandage for repair of a complicated radial artery pseudoaneurysm.Ann Vasc Surg. 2014; http://dx.doi.org/10.1016/j.avsg.2013.12.006PubMed Google Scholar The clinical course was complicated by skin ulceration. Despite this complication, conservative treatment was not abandoned and complete thrombosis of the pseudoaneurysm eventually occurred within 4 weeks of treatment, preserving arterial patency. We have concluded that arterial pseudoaneurysms of the upper extremity are likely to thrombose more often than currently believed. Therefore, given the potential for such an artery-sparing treatment option, clinicians should be encouraged to consider the nonoperative approach as the first-line treatment for these relatively uncommon vascular lesions. They reference the case of a hemophiliac patient with a radial artery pseudoaneurysm, which was safely treated with compression bandages despite transient development of skin ulceration.1 This case and our original case report2 nicely demonstrate that traumatic pseudoaneurysms of the upper extremity can be successfully managed nonoperatively in children.
