- W2105708420 abstract "Background and Aim: In our previous report, patients with the gallbladder (GB) in situ with stones were identified as the highest risk of recurrent common bile duct (CBD) stones after removal of CBD stones using endoscopic papillary balloon dilation (EPBD) (Tsujino T et al. Clin Gastroenterol Hepatol 2007). The aim of this study was to determine the predicting factors for the development of CBD stone recurrence after EPBD in this group of patients in the single center study. Patients and Methods: Bewteen May 1994 and July 2006, a total of 144 patients had GB stones in situ after complete removal of CBD stones by EPBD alone. Detained analyses including imagings were performed in 140 (97%; 62% male; mean age 68 years) of 144 patients. The risk of stone recurrence was evaluated and the risk factors for stone recurrence were analyzed by univariate and multivariate analysis. Results: During the mean follow-up period of 3.8 years (range, 0.1-11.9 years) after EPBD, 30 (21%) patients developed CBD stone recurrence. Mean interval between EPBD and recurrence was 22.9 months (range, 0.7-74.7 years). Recurrent stones were extracted endoscopically in all cases. The cumulative stone recurrence rates at 1, 3, and 5 years after EPBD were 11%, 24%, 30%, respectively. Univariate analysis identified the following 3 risk factors for stone recurrence; the diameter of CBD ≧10 mm, the diameter of the cystic duct ≧4 mm, and the use of mechanical lithotripsy. By multivariate analysis, however, only the diameter of the cystic duct ≧4 mm was found be statistically significant (Odds ratio 2.90, 95% CI 1.112–7.894). Conclusions: The incidence of stone recurrence is considerably high in patients with GB stones after EPBD. Because patients who have the dilated cystic duct are at increased risk for developing stone recurrence, careful follow-up is required in this subset of patients after removal of CBD stones by EPBD." @default.
