- W2106981946 abstract "Worldwide more than 450 million people have a mental or behavioural problem. Almost one million people commit suicide every year. Depression, alcohol-use disorders, schizophrenia and bipolar disorder are among the six leading causes of years lived with disability; one in four families have at least one member with a mental disorder (WHO 2001a). The overwhelming majority of people receive no treatment or treatment which is inadequate or even harmful (WHO 2003). Nurses have an important role in overcoming barriers to effective treatment. However almost half of the world's population has access to less than one psychiatric nurse per 100 000 population. There is also marked regional imbalance with South-east Asia having a median number of 0.16 psychiatric nurse per 100,000, compared to 27.5 in the European region (WHO 2001b). Research is required urgently to guide the development of much needed services, treatment and care for people with mental disorders, especially in low and middle income (LAMI) countries. Unfortunately nursing research in mental health is limited and represents only a small proportion of nursing research publications (Borbasi et al. 2002; Traynor & Rafferty 2000). In LAMI countries, nursing research in mental health is even more limited. Scientific journals play an important role in the publication and dissemination of research. Late last year members of the World Health Organization's Department of Mental Health and Substance Abuse met with editors of scientific journals publishing mental health research. The aim was to generate ideas for increasing research publications from LAMI countries. Nurse researchers from LAMI countries are often unable to meet the requirements of journals because they have limited access to information, lack advice on research design, have difficulty writing in a foreign language and experience financial, administrative and infrastructure constraints. In most LAMI countries there is no specialist mental health nursing journal. Indeed the high priority of promoting mental health in primary care necessitates that nursing research in mental health should also be disseminated via generalist nursing publications. Unfortunately, the lack of priority given to mental health by many governments, nursing institutes and nursing journals restricts opportunities for the publication of such research. The International Nursing Review should be congratulated for including three articles on mental health in this issue, one of which originates from Brazil (a middle income country according to World Bank criteria). These articles illustrate the breadth of issues associated with nursing and mental health including self harm among young people, drug and alcohol misuse in Brazil and monitoring adverse psychotropic drug reactions. Practical suggestions for promoting nursing research in mental health and supporting authors from LAMI countries include: prioritizing mental health in generalist nursing journals; educating reviewers about the research needs and infrastructure of LAMI countries; sensitizing readers and other stakeholders to the role of nursing issues in mental health care in LAMI countries through special issues or guest editorials; including reviewers and correspondents with special interest and expertise in LAMI countries; promoting a multilingual approach and supporting the translation of articles into regional and national languages; educating researchers about the peer-review process; ensuring that authors receive detailed constructive feedback; and providing for extra rounds of reviewing and technical assistance with language. A major barrier to accessing nursing research in mental health from LAMI countries is the lack of nursing journals published in these countries. The journals that are available are often not indexed in international databases, published with limited resources and have a restricted author pool. Editors of nursing journals in LAMI countries may require support to improve the standard of the journal and increase the number of research publications on mental health. Strategies such as experienced editors from high income countries participating on editorial boards, providing mentorship, establishing twinning arrangements between journals and conducting training workshops will assist the development of nursing journals in LAMI countries. While many nursing journals have a wide distribution, the majority of subscribers are in high income countries. Special attention needs to be given to the dissemination of nursing research in mental health in LAMI countries to maximize its impact on mental health policy and practice and to encourage the development of a research culture among nurses working in mental health. Increasing the online availability of journals is cost effective. Initiatives such as the WHO-led Health InterNetwork Access to Research Initiative (HINARI) offer institutions in LAMI countries access to thousands of journals at no or very low cost. More than 80 nursing journals, both generalist and mental health specific, can be accessed from this collection. While many barriers still exist, for example, language and lack of access to computer technology, open access strategies increase the possibilities for nurses to access and utilize mental health research. Priorities for nursing research in mental health from LAMI countries include mental health promotion, prevention, psychosocial interventions, mental health and primary health care, community mental health care, acute and intensive care for people who are severely mentally ill, consumer satisfaction, quality of life, adherence to treatment and rehabilitation. Such research will assist in understanding the factors that influence the causes, course and outcome of mental disorders and the variations across different communities and facilitate effective care in settings where resources are scarce. Nursing journals and professional organizations such as WHO and the International Council of Nurses can promote nursing research in mental health from LAMI countries and facilitate its dissemination to all nurses.
