- W2107401299 abstract "Objective: Clinical interventions for the upper extremity often focus on motor training with minimal attention to sensory training. We developed an Upper Limb Training and Assessment (ULTrA) telerehabilitation program that combines sensory and motor activities with the purpose of improving function. This abstract focuses on the impact of the ULTrA program on proprioception. Design: Intervention study with pre/post tests. Setting: Pre/posttests were conducted in a research lab. Participants completed the ULTrA program at home. Participants: 12 adults (34y±10y) with hemiplegic cerebral palsy (CP). Interventions: Individualized task specific, upper-limb training included fine motor tasks, stereognosis, tactile discrimination, and object manipulation. Training was 5 times per week, 40min/d for 8 weeks, with task difficulty increasing as tolerated. Participants were coached by a student trainer via webcam. Main Outcome Measures: Tactile assessments: (1) sensory domes - pre/post testing, (2) braille module - ULTrA program. Position sense assessments: participant reproduced elbow joint position with the same (ipsilateral remembered - IR) or opposite arm (contralateral remembered - CR). Results: At baseline, participants were more accurate with the less affected (LA) hand in both tactile assessments. Following training there was symmetry between hands with the more affected (MA) hand performing as well as the LA hand. In position sense, there was no difference between arms in IR. In the task requiring hemispheric transfer (CR), the MA arm showed greater accuracy in matching LA arm position. Following training, the ability of the LA arm to match position of the MA arm significantly improved. Conclusions: Sensory performance following training improved whether testing occurred on trained or untrained tasks. Improved performance on CR position matching tasks suggests that training affected interhemispheric transfer of sensory information. Together, these findings demonstrate that propriceptive ability can be improved with sensorimotor training." @default.
