- W2107647059 abstract "The aim of the study was to test a new multiparameter scoring system, pelvic masses score (PMS), for the ovarian malignancies identification and to evaluate its accuracy. 102 pelvic masses were evaluated according to Sassone's score, Ovarian Tumor index (OTI score) and risk of malignancy index (RMI score) considering pulsatility and resistance indexes, volume of the masses, echogenicity, location of tumour vascularity, menopausal status and CA 125 serum levels. The results of pathological examination were obtained for each mass. A preliminary explorative analysis let us to identify prognostic factors to combine for a new multiparameter scoring system. The PMS score was tested for specificity, sensitivity, positive and negative predictive values. The Receiver Operating Characteristic (ROC) method let us to compare the diagnostic ability of Sassone OTI, RMI and PMS scores. 32 (31%) masses were malignant and 70 (69%) were benignant. PMS is a multiplicative score of resistance index, Sassone's score and location of tumour vascularity, properly weighted for menopausal status. The Sassone's score sensitivity was 69%, specificity 93%, positive predictive value 81% and negative predictive value 87%. The accuracy was 85%. The area under the ROC curve was 0.93 C.I. 95% [0.88;0.98]. The RMI score sensitivity was 78%, specificity 88%, positive predictive value 75% and negative predictive value 90%. The accuracy was 85%. The area under the ROC curve was 0.93 C.I. 95% [0.88;0.98]. The OTI score sensitivity was 90%, specificity 65%, positive predictive value 54% and negative predictive value 94%. The accuracy was 73%. The area under the ROC curve was 0.83 C.I. 95% [0.75;0.91]. The PMS sensitivity was 91%, specificity 90%, positive predictive value 93% and negative predictive value 96%. The accuracy was 89%. The area under the ROC curve was 0.97 C.I. 95% [0.95;0.99]. The PMS score demonstrated an accuracy higher than the other scores considered. So this new score could be proposed as a valid alternative method to use in gynecology clinics." @default.
