- W21080913 abstract "The principles for medical management of transient myocardial ischemia in patients with angina pectoris are based on drugs that affect either myocardial oxygen demand, myocardial oxygen supply or both, as the development of symptomatic or asymptomatic myocardial ischemia is caused by an imbalance between myocardial oxygen demand and myocardial oxygen supply. Traditionally, a fixed coronary artery stenosis has been assessed in most patients with stable angina pectoris as the cause of transient myocardial ischemia through an increase in myocardial oxygen demand and with a fixed coronary flow reserve. Hitherto, this concept has been supported by exercise testing, which often is very reproducible with the same level of work load and with the same level of myocardial ischemia in individual patients. However, with the use of ambulatory electrocardiographic monitoring it has been noted that transient myocardial ischemia during daily life in patients with stable effort-induced angina pectoris occurs at a heart rate level which is substantially lower than the heart rate at exercise-induced myocardial ischemia [6, 8, 13, 21, 62, 75, 85]. This observation makes the assessment of transient myocardial ischemia during daily life more complex and a reduction in myocardial oxygen supply, which is normally tolerated during exercise testing, seems to play an essential role even in classic exertional angina. It has in this context been observed, that patients with stable angina may exhibit episodic vasoconstriction superimposed on fixed atherosclerotic lesions induced by increased alpha-adrenergic stimulation by a cold pressor test [50]." @default.
