- W2108568045 abstract "In the late 1960s and early 1970s, new techniques of identifying and quantitating ventricular premature complexes (VPCs) were developed, in parallel with a better understanding of the use of antiarrhythmic drugs, the development of new drugs, and increasing attention to the feasibility of preventing potentially lethal arrhythmias. It was in this atmosphere in 1971 that Lown and Wolf 1 proposed their widely used ventricular arrhythmia classification system. The pragmatic simplicity of this grading system favored its rapid expansion to general use in many clinical and research settings, where it has remained a standard. Even though data from a number of the major ventricular arrhythmia studies (see summary by Winkle 2) could not adapt comfortably to this standard classification, no modification or new system of classification has been proposed. However, the need for a modified formulation was evident in data management, in that many of the studies cited by Winkle 2 labeled chronic VPCs as high risk, either by predetermined arbitrary hypotheses or by retrospective derivation from data. Recently, Bigger and co-workers 3-5 suggested a need for a systematic modification of arrhythmia classification. Any classification that is to be more than merely descriptive must respond to the quantity and quality of data available from new technology, and to current methods of assessment and interventions for ventricular arrhythmias. Information now available suggests that a flexible classification system, in which of VPCs and of ventricular arrhythmias are assigned to separate but parallel hierarchies, may address the needs of both the clinician and the investigator.
